Comprehensive Child Home Visit Assessment Tool
The PatchSA Home Visit CARES Score provides guidance for health care providers who must evaluate the level of interventions needed for a child with a life-limiting or life-threatening illness.
Developed specifically for South Africa, the CARES Score provides a useful tool for those who visit children in their homes to ascertain the child’s immediate needs.
CARES stands for Comfort, Access (to a medical facility), Resources, Emotional Needs and Safety – Each of which is evaluated and scored according to three levels indicating whether additional interventions are needed.
